Family/Educational Leave Request Forms should be submitted at least two (2) weeks prior to the trip explaining the reason for the request and duration of the trip. Approval forms are available on-line as well as in the parent handbook.
Please fill out this form for online excuse form submission. Absence excuses must be returned within 3 days upon returning to school. It is the student's parent(s) responsibility to ensure that all excuses are turned in a timely manner.
